Transitions and changes are part of everyone’s life. The vast majority of children and young people look forward to moving on. However, transitions can be challenging and support from parents and staff can help transitions go more smoothly. Education Scotland
This Transition Planning Tool could be used to support enhanced transitions for children with Additional Support Needs. It contains a selection of topics for staff and parents to consider, plan for, and review in the months leading up to a vertical transition e.g. ELC into Primary 1, changing classes or schools, or moving into secondary education.
Templates for sharing personalised information
In Early Learning and Childcare settings The Highland Council recommend the use of Being Me! My Personal Care Plan which is a valuable part of information sharing at times of transition. For more information about this document see the ELC Transitions page.
Know Me Support Me is based on the All About Me section of Being Me! but can be used across other primary transitions.
Photos, videos, and digital books can be useful to support a child by giving them visual information about the different spaces within a new setting and the people who work there. They can act as a visual reminder during the time between visits and starting at the setting. Photo books can be printed or created digitally using something like Book Creator. Digital versions could contain photos and video clips and be shared online. This is an ideal way to create an introduction and virtual tour of your setting.
