Transition is considered to be an adjustment over time to new contexts
outside the family, where babies and children experience changes to their
social environment, to their routines, to what is expected of them and to the
relationships they have with others in new situations. Our aspiration should
be to promote in babies and children a sense of belonging by providing
them with their entitlement to continuity of learning in a caring and respectful

Being Me!

The Early Years Team in The Highland Council recommend the use of Being Me! My Personal Care Plan. Sections in Being Me include: My Profile, All About Me, and Meeting My Ongoing Needs. There is scope within the All About Me section to provide personalised information relating to a child’s ASN which is a valuable part of information sharing at times of transition.

There may be occasions where additional information needs to be planned for and shared over and above the Being Me! document. Templates for transition planning and other supporting documents can be found in the section below. Not all of them will be relevant to every child but choose the ones which are most relevant to your situation.

The Enhanced Transition ELC Planner is designed to support transitions from home into an ELC setting for children with ASN. It can be used by staff when planning this transition with parents/carers. Templates for sharing personalised information

Photos, videos, and digital books can be useful to support a child by giving them visual information about the different play spaces within a new setting and the people who work there. They can act as a visual reminder during the time between visits and starting at the setting. Photo books can be printed or created digitally using something like Book Creator. Digital versions could contain photos and video clips and be shared online. This is an ideal way to create an introduction and virtual tour of your setting.
